President, deputy and health minister all on medical leave

Ramaphosa was booked off on Wednesday after returning from the BRICS Summit. No acting President has been appointed, and Ronald Lamola will lead the delegation to the UN General Assembly.

Ramaphosa ill health: who stands in | Southafriworld

What the Presidency said

The Ramaphosa ill health announcement came from the Presidency on Wednesday afternoon, 16 September 2026, two days after the President returned from the 18th BRICS Summit in New Delhi.

“Due to ill health, President Cyril Ramaphosa is currently booked off from public engagements,” said Vincent Magwenya, Spokesperson to the President, adding that Ramaphosa had been advised by his medical team to rest and recover.

The practical arrangement is that ministers step in at individual events. Where possible, the President has asked the respective ministers to represent him at scheduled public engagements.

Magwenya said Ramaphosa remains in good spirits and continues to monitor government work remotely. “He thanks South Africans for their well wishes” and looks forward to resuming his public programme soon, the spokesperson said.

Ramaphosa attended the summit, held on 12 and 13 September under India’s chairship, and landed back in South Africa on Monday 14 September. He is 73 and turns 74 in November.

What the Ramaphosa ill health statement says and does not say

The Presidency has not disclosed the nature of the illness, and has not indicated how long the President will be away from public duties. That is the Presidency’s call to make, and no inference about his condition can be drawn from the statement.

What the statement does establish is the scope of the arrangement. Ministers are representing the President at public engagements. That is not the same as a transfer of executive authority, and the Presidency has not announced any acting President arrangement.

Magwenya’s fuller comment describes the sequence plainly. The President returned from the summit and was not feeling well, his medical team advised him to rest, and he has been booked off from public engagements until he has fully recovered.

Three senior offices on medical leave at once

Mashatile issued his own update on Tuesday, 15 September, saying he had recently undergone a minor medical procedure, that his recovery was progressing well and that he was regaining strength. Ramaphosa visited him at his Johannesburg home in August and said afterwards that he was pleased with the recovery.

The Health Ministry has been under an acting arrangement since Motsoaledi went on sick leave, with Ndabeni-Abrahams appointed to act until 30 September.

Three senior offices being under medical arrangements in the same month is unusual, and it has left ministers carrying more public engagements than normal. None of the three departments has said the arrangements affect decision-making, and the Presidency’s statement describes representation at events rather than any change in who exercises authority.

Lamola goes to New York instead

The clearest consequence is the United Nations General Assembly. The South African Broadcasting Corporation reported that Ronald Lamola, the Minister of International Relations and Cooperation, will lead the delegation in Ramaphosa’s absence.

That sends Lamola to the United States days after a sequence neither government planned around.

On 15 September, United States Secretary of State Marco Rubio announced a visa restriction policy targeting individuals the State Department determines enact or enable policies it describes as race-based discrimination, land confiscation without compensation and incitement of racial violence in South Africa. On Wednesday, Lamola issued South Africa’s response, calling the announcement a mischaracterisation of South African policy and asserting a sovereign right to make the country’s own laws.

He now leads the delegation to the General Assembly, in a country whose government announced the restrictions and to which he has publicly objected. No United States response to his statement had been published at the time of writing.

The commercial relationship remains a separate track, including the African Growth and Opportunity Act extension to December 2028 and the tariffs applied to South African goods, which Southafriworld has covered in its reporting on American companies operating in South Africa.

What has not been said

No return date has been given for the President. The Presidency said only that he is booked off until he has fully recovered.

No acting President has been appointed, and the Presidency has not said whether one is contemplated. Nothing in the statement suggests the President is unable to exercise the functions of the office, and the arrangement described is limited to public engagements.

The Presidency has not listed which engagements are affected or which ministers will take them. It has not said whether Cabinet meetings or scheduled sittings will proceed as planned.

No further update on Mashatile’s return to full duties has been given beyond his own statement of 15 September, and the acting arrangement at Health runs to 30 September unless extended.

The General Assembly’s high-level week in New York is the next fixed point on the diplomatic calendar, and South Africa’s delegation will be led by its foreign minister rather than its head of state.

i HOW WE REPORTED THIS
Researched
Published
  • The article reports what the Presidency said and states clearly what it did not say. The nature of the President's illness has not been disclosed and Southafriworld does not speculate about it, does not describe symptoms, and draws no inference from his age.
  • The article distinguishes between ministers representing the President at public engagements, which is what the Presidency described, and any transfer of executive authority, which the Presidency has not announced. It does not assert that any constitutional acting-President provision has been triggered.
  • The observation that three senior offices are under medical arrangements in the same month is assembled from three separate published statements, each attributed and dated. The article notes that none of the departments has said the arrangements affect decision-making.
  • The United Nations General Assembly delegation change is attributed to the South African Broadcasting Corporation as reported by Reuters, because the Presidency's statement does not mention it.
  • No comment was sought from the Presidency, the Deputy President's office or any department for this article. Every quotation used was already on the public record.
Corrections and updates: Corrections and material updates are disclosed at the end of the article with a dated note. This is a developing story and the position reported here reflects Wednesday, 16 September 2026.
